Member Registration Instructions

• Enter your 4-digit member number (e.g., 0042).
• First Name: Enter your first name and middle initial exactly as it appears on your statement (must include any prefix, spaces or punctuation shown).
• Last Name: Enter your last name and credentials exactly as they appear on your statement. (must include any spaces, suffix, professional credentials or punctuation shown).

When you have completed all three fields, click Validate. You will be directed to the next page where you will choose a Username and set your Password.
